The P3 Program™ Process of Care is an evidence-based therapeutic process delivered by trained pharmacists for the management of patients with chronic disease(s). This process is called Comprehensive Medication Therapy Management (CMTM) services, which is an interactive P3 clinical service guided by a propriety health information technology, ThinkEHRx™. P3 Program™ CMTMservices have been studied and proven to improve the health status of patients while reducing unnecessary costs of care.
