Objectives: Registration information: General Registration Fee is: $25 (includes continuing education credit, handout materials and light refreshments) Register online with your credit card at www.pharm.buffalo.edu/ce (your username is your email address) To Register by mail, please send your registration and check (payable to the UB Foundation, Inc.) to: CPE Office, 204 Kapoor Hall, UB School of Pharmacy & Pharmaceutical Sciences, Buffalo, NY 14214. We are unable to accept cash payments for program fees. In order to successfully participate from a remote location via Webinar, you must possess a computer with a high speed connection (not dial-up), external speakers or a headset. You will be able to view the presentation, hear the speaker and address questions via the chat function. Full instructions with the WebEx URL, along with a PDF of the presentation handouts will be emailed to you two days before the program. After the conclusion of the Webinar session you will receive an online evaluation for you to complete. How to Register for CPE Monitor: Pharmacists are required to obtain their NABP e-profile ID and to set up their CPE Monitor account. Please visit www.mycpemonitor.net to set up a CPE Monitor account. Be sure to have completed this process in order to guarantee receiving your CE credit. Setting Up Your UB CE Account - Next, please create your UB CE online account (you only need to do this once) if you have not already done so at: www.pharm.buffalo.edu/ce so you can register online for our programs and we can upload your credit to your CPE Monitor account post program Please Note: ACPE has instituted a 60 day time limit on uploading CE credits to the CPE Monitor. Be sure that your information is legible and correct (including month/date of birth and NABP number) and that you sign your attendance form and turn in the completed form at the end of the program. If there are any issues with uploading your credit to the CPE Monitor the CE office will contact you via email. All notifications are sent through the pharmacyce@buffalo.edu email address. Be sure post-program, you check your CPE Monitor to be sure all program credits appear. Once it is sixty days past the program date the CPE Monitor will no longer accept credit uploads for that program. Cancellation Policy: We reserve the right to cancel this program due to such factors as insufficient registration. Registrants will be notified in this case. Requests for refunds, minus a $20 service charge, must be in writing and will be honored up to one week prior to the program. After that time, no refunds will be made. Parking: Parking Permits are not required in the Parker lot. Turn left at first light and enter the UB Campus. Take immediate left into Parker Lot. From Amherst, Williamsville, Niagara Falls: Take 290 to Millersport Highway South. Continue on Millersport Highway and veer left on Bailey Avenue. Turn right on Main St. (Rt. 5). Turn left on Winspear Avenue. Turn left at first light and enter the UB Campus. Take immediate left into Parker Lot.
