Team Up Against MS: The Multidisciplinary Patient-Centered Team Approach to Multiple Sclerosis

Target Audience
This program is designed for neurologists and MS specialists including physicians, nurse practitioners, physician assistants, and nurses.

Program Overview
Patients with multiple sclerosis (MS) often need the intervention of multiple specialists, resulting in a complex organization of care. This CME/CE podcast features an interprofessional team of healthcare professionals from the MS Comprehensive Care Center at Holy Name Medical Center who will share their insights on best practices for patient-centered care of people with MS. The MS Care Team will share key aspects of their evolution as a team using two cases of patients with MS. This discussion will highlight how they 1) evaluated existing practices, 2) identified areas of improvement, 3) prioritized and implemented changes in care, and 4) assessed the team’s progress toward achieving their patient-centered care goals.

Learning Objectives
Upon completion of this activity, participants should be better able to:

DIAGNOSE and subtype patients with MS based on clinical history, MRI findings, and updated McDonald criteria
ASSESS MS disease activity and progression longitudinally
RECOGNIZE suboptimal treatment response and optimize therapy, with the goal of minimizing disability
DISCUSS MS disease course, treatment options, and quality-of-life concerns with patients and their families
