Comprehensive healthcare interventions increasingly show positive reductions in chronic disease. Patients live healthier lives by keeping cholesterol, blood pressure, and glucose at moderate levels, engaging in daily physical activity, eating a well-balanced diet, maintaining a healthy weight, and avoiding tobacco products.
Optimal management of patients with chronic diseases and unhealthy lifestyle behaviors requires strategies that support better patient self-management. These strategies demand collaborative care planning, ongoing monitoring and patient education. By keeping pace with advancements in medical knowledge, therapies and technologies, primary care clinicians can best manage the health of patients with chronic disease.
Improving Health Outcomes in Chronic Disease provides primary care clinicians with practical approaches to improve care outcomes for patients with chronic conditions. This program offers practical, case-based lectures, time for dialogue and “how-to” approaches to patient assessment and management.

Educational Objectives:
Following this learning activity, participants will be able to:
- Apply clinical advances for long-term management of common chronic conditions including anxiety, depression, diabetes, edema, fibromyalgia, heart failure and osteoarthritis
- Discuss behavioral, environmental and socioeconomic factors that contribute negatively to health
- Integrate into clinical practice recommended communication guides and approaches to behavioral health
