The goal of this continuing education program is to educate attendees about the management of diabetes across the patient care continuum, including the inpatient and outpatient settings, and safe transitions of care with medications and monitoring devices. By attending this seminar, participants should be able to: 1. Recognize common medication safety issues related to diabetes management in both the inpatient and outpatient settings.* 2. Identify at least three error-reduction strategies to ensure safe use of insulin in the management of hyperglycemia in hospitalized patients.* 3. Discuss strategies for overcoming clinical inertia in the management of hyperglycemia in both the inpatient and outpatient settings. 4. Analyze factors that impact outpatient diabetes management decisions, from Cardiovascular Outcomes Trial (CVOTs) data to social determinants of health. 5. Discuss advancements in self-monitoring blood glucose (SMBG) and continuous glucose monitors (CGMs). 6. Explain how pharmacists can ensure safe care transitions for patients as they move to and from the inpatient and outpatient settings.* *Patient safety focus |
