The P3 Process of Care: Comprehensive Medication Therapy Management

Overview / Abstract:

The P3 Programâ„¢ Process of Care is an evidence-based therapeutic process delivered by trained pharmacists for the management of patients with chronic disease(s). This brief model prepares pharmacists for new roles in clinical care using the P3 process called Comprehensive Medication Therapy Management (CMTM) services. This interactive, patient-centered clinical service provides a framework for practitioners to improve the health status and self-efficacy of their patients. P3 Programâ„¢ CMTMservices have been studied and proven to improve the clinical health outcomes of patients while reducing unnecessary costs of care. Learners will be introduced to the six-steps in the process with emphasis in the use of the Therapeutic Experiment to promote pharmacist-provided disease management.

Learning Objectives

At the end of this application-based activity, participants should be able to:
Apply the six-step P3 Programâ„¢ model of care for the management of patients with chronic diseases;
Reflect on the changes to the pharmacists role as a provider of CMTM; and
Identify resources and tools to conduct comprehensive medication therapy management.
