Preventing Medical Errors for Clinical Health Professionals

Overview / Abstract:

Needs statement
The 1999 Institute of Medicine report To Err is Human: Building a Safer Health System brought prominence to the issue of medical error in the US. Since that time multiple activities at various levels of private and regulatory arenas have targeted identification and resolution of medical error. The process is dynamic and health professionals need education of error reduction techniques so as to improve safety in their professional activities and work setting. Some regulatory agencies require specific medical error education for licensure or re-licensure of health professionals.

Course goals
This educational monograph addresses the identification, reporting, monitoring and reduction of medical error. It is intended to make participants aware of the magnitude of the problem, of the many-leveled activities needed to resolve it, and the importance of each person’s role in the national effort to reduce medical errors.

Target audience
This activity is designed for clinical health professionals including physicians, pharmacists, nurses and psychologists at all levels and settings in the healthcare industry.
Learning objectives

At the completion of this course, the learner should be able to:
1. Recognize the magnitude and far reaching effects of medical error;
2. Categorize factors contributing to the occurrence of medical error;
3. Describe the healthcare professional’s role in identifying error prone situations;
4. Explain the importance of reporting medical error;
5. Specify at least two recommended practice modifications to promote safety and reduce the incidence of medical error;
6. Review the processes of root cause analysis and failure mode and effects analysis when analyzing error.

Mary Kathleen Ebener PhD, RN
Dr. Ebener teaches courses in nursing, healthcare and statistical analysis at the Florida State College, Jacksonville, FL. She holds a BSN from Illinois Wesleyan University; her MSN is from Andrews University, MI; and her PhD. is from the University of Florida, Gainesville. Peggyann Zaenger, B.S., PharmD
Dr. Zaenger has a Bachelor of Science in Pharmacy degree from the University of Toledo and a Doctor of Pharmacy degree from the University of Michigan. She previously worked at AKH Inc. as an Educational Consultant. Dr. Zaenger discloses no significant financial relationships with pharmaceutical or medical product manufacturers.

Jonathan S. Holman, B.S., Pharm. D.
Dr. Holman has extensive experience as a clinical pharmacist in adult intensive are and has held clinical management roles in medical centers. He currently serves as adjunct faculty for the University of Florida and supervises a clinical rotation in Drug Information. In addition to his role at AKH Inc., Dr. Holman works as a clinical pharmacist at Orange Park Medical Center.

Dr. Holman holds a Bachelor of Science degree in pharmacy from the University of Colorado and a Doctor of Pharmacy degree from the Medical university of South Carolina. He is a certified CME Professional from the National Commission for Certification of CME Professionals, Inc. AKH REVIEWER
Christine Redfield, B.S, Pharm. D.
Dr. Redfield has experience in clinical, consulting and pharmacy education. She currently holds her pharmacist licensure in the State of Florida and Oregon and is a registered Pharmacy Consultant in Florida. After completing her pharmacy residency in the State of Wyoming, her focus became strong in geriatrics, behavioral health and education. She is currently working as a Clinical Pharmacist in a hospital in Southern Oregon as well as participating as an independent consultant.

Dr. Redfield holds a Bachelor of Science degree in both Chemistry and Biology from the University of North Florida. She also holds a Doctorate of Pharmacy degree from the University of Florida. She has extensive experience in the research field, having held a position with The Mayo Clinic, Jacksonville. She was the 2009 recipient of the Judith Reyes Award, has given numerous presentations covering various pharmacy topics and is a volunteer for the Medical Reserve Corps.
