Conference Overview:
Our aging population is expected to grow to almost 87 million by 2050. Over the last decade, the fastest growing cohort has been individuals 85 years and older. With people living longer and older, this conference will provide clinicians and chaplains an update on the latest evidence-based concepts in managing life-limiting illness and end-of-life care. Topic highlights for physicians, physician assistants, nurse practitioners, nurses, social workers, psychologists and chaplains will include treatment options, pain management, medical ethics, goals of care, quality of life, palliative and hospice care, and bereavement and grief.

Educational Objectives:
Following this learning activity, participants will be able to:
- Describe the impact of chronic illness on life expectancy
- Define the purpose of the Physicians Orders for Life Sustaining Treatment (POLST)
- Identify treatments to manage pain and non-pain symptoms in palliative care and at the end of life
- Describe strategies to structure and facilitate a patient-family-facility care conference, including how to set care goals and resolve family conflicts
- Present medical-legal-ethical treatment options at the end of life
- Discuss the hospice benefit and educate patients and families about palliative care
