In this interactive infographic format, Monica Peek, MD, MPH, MSc, uses her clinical and community experience to provide perspectives on disparities in diabetes care. Using videos, audio, and animated content, this highly visual, self-directed platform creates a practical framework to translate awareness of patient, clinician, and macro-level contributors to disparities into clinical care. Strategies are discussed on how to implement change at an individual and practice level. The activity additionally provides patient insights on treatment challenges and goals, and the importance of shared decision making within the multidisciplinary team. |
