NARRATIVE DESCRIPTION
Following this course, the participant should be able to appraise the etiologies and recognize the clinical presentations of the commonly seen disorders presented; indicate diagnostic tests available and the suggested use for making a diagnosis; implement available treatment options and appropriate long-term care when indicated. This activity is expected to result in improved competence in making an appropriate diagnosis and providing effective treatment and referral or follow-up care with the overall goal of improving patient outcomes.

When indicated in the specific objectives, emphasis will be on aligning physician behavior with current guidelines and evidence-based medicine with a focus on diagnosis, treatment and when to refer. This course was designed to inter-relate the disorders presented in an effort to help all practitioners at the level of a practicing physician or practitioner relate their practice strategies with other providers and specialists. By doing this, it will provide them with increased competence to appropriately diagnose and refer or effectively treat their patients’ conditions to optimize their care.

SPECIFIC OBJECTIVES
Day 1
Use and Interpretation of Thyroid Tests. (Muncie)
Upon completion of this session, using the American Thyroid Association Guidelines, the Cochrane Abstract Database and current literature, the participant should be able to: EBM, GL, COMP
1. Integrate the serum tests for thyroid function.
2. Interpret thyroid tests results that occur in clinical settings.
3. Recommend the appropriate follow-up testing of abnormal thyroid tests and the treatment of these thyroid abnormalities.
4. Appraise the need to treat subclinical hypothyroidism or subclinical hyperthyroidism.
5. Recommend the evidence-based evaluation and treatment of thyroid nodules.

Delirium in the Elderly - Current Concepts. (Oberlender)
Upon completion of this session, the participant should be able to: EBM, COMP
1. Specify diagnostic features and common causes of delirium in seniors.
2. Distinguish the causes of persistent delirium.
3. Use evidence-based medicine to formulate treatment approaches to delirious seniors.
4. Recognize treatment options for intermittent agitation using evidence-based medicine.

Evaluation and Treatment of Hypertensive Patients. (Muncie)
Upon completion of this session, using the most recent JNC Guidelines, the Cochrane Abstract Database, the evidence-based reports from Clinical Evidence and the current literature, the participant should be able to: EBM, GL, COMP
1. Describe the issues that complicate the diagnosis of hypertension and the way to avoid them.
2. Employ the non-pharmacologic treatment options to manage hypertension.
3. Specify the pharmacologic treatment options for patients with compelling indications based upon JNC Guidelines and current literature.
4. Formulate the therapeutic goal for controlling blood pressure in different patient populations and the combination of medications that can achieve that goal.
5. Distinguish the side effect risks for each class of antihypertensive medications.

Evaluation and Treatment of Dementia in Seniors. (Oberlender)
Upon completion of this session, the participant should be able to: EBM, COMP
1. Determine the common causes of cognitive dysfunction and dementia in seniors.
2. Develop an evidence-based evaluation of seniors with cognitive dysfunction.
3. Evaluate the relative risks and benefits of available treatment options for dementia in seniors.
4. Distinguish the central role of caregiver education and support in treating seniors with dementia.

Treating Patients with Type 2 Diabetes Mellitus. (Muncie)
Upon completion of this session, using the American Diabetes Association, the European Association for the Study of Diabetes Guidelines and the Cochrane Abstract Database, the participant should be able to: EBM, GL, COMP
1. Describe the diagnostic criteria for type 2 diabetes.
2. Evaluate the therapeutic goals in treating type 2 diabetes and their impact on morbidity and mortality.
3. Recommend the approach for choosing the most appropriate hypoglycemic agents to treat type 2 diabetes.
4. Specify the clinical situations that would require the use of insulin to treat type 2 diabetes and the probable dosage form and average amounts required for control of A1c.
5. Develop the therapeutic options for preventing diabetic complications.

Day 2
Adult Health Screening and Immunizations – Evidence-Based Approach. (Muncie)
Upon completion of this session, using the USPSTF and Cochrane Abstract Database, the participant should be able to: EBM, GL, COMP
1. Apply the evidence-based recommendations for preventive screening tests for adults.
2. Determine the immunizations required for adults.
3. Appraise the side effects of adult immunizations.
4. Select the contraindications for each adult immunization and their alternative therapy.
5. Plan when screening tests can be discontinued in adults.

Sensible Approaches to Geriatric Infections. (Oberlender)
Upon completion of this session, the participant should be able to: EBM, COMP
1. Recognize atypical presentations of infectious illness in the elderly.
2. Differentiate the causes of pneumonia and UTI in seniors.
3. Specify an evidence-based approach to appropriate antibiotic usage.
4. Appraise evidence-based considerations in emerging antibiotic resistance.
5. Apply evidence-based options in the prevention and treatment of C. difficile illness in seniors.

Chronic Obstructive Pulmonary Disease (COPD): Evaluation and Treatment. (Muncie)
Upon completion of this session, using the ACCP GOLD guidelines, the AACVPR practice guidelines and the evidence-based reports from Clinical Evidence, the participant should be able to: EBM, GL, COMP
1. Appraise patient specific data to determine if they meet diagnostic criteria for COPD.
2. Describe the criteria for each stage of COPD.
3. Formulate a treatment plan for the stages of COPD.
4. Recommend the follow-up required to assess the therapeutic efficacy of the treatment options.
5. Formulate an evidence-based treatment plan for the evaluation and treatment of a COPD exacerbation.
Geriatric Depression: Advances in Diagnosis and Treatment. (Oberlender)
Upon completion of this session, the participant should be able to: EBM, COMP
1. Differentiate the variety of clinical presentations of depressive disorders in seniors.
2. Appraise the utility of, and plan to use, the Geriatric Depression Scale.
3. Apply an evidence-based approach to antidepressant drug therapy in seniors.
4. Apply the evidence-based recommendations for use of non-drug antidepressant therapy.
Recurrent Headaches – Update on Diagnosis and Treatment. (Muncie)
Upon completion of this session, using the International Headache Society and U.S. Headache Consortium Guidelines and the National Guideline Clearinghouse and the evidence-based reports from Clinical Evidence, the participant should be able to: EBM, GL, COMP
1. Construct the approach for evaluating a patient with recurrent headaches with an emphasis on possible migraine headaches.
2. Assess the advantages of supportive therapy and lifestyle changes in treating migraine headaches compared to pharmacologic therapy.
3. Appraise the pharmacologic options for treating the acute pain of a migraine headache.
4. Recommend a prophylactic therapy regimen for patients with recurrent migraine headaches.
5. Describe the diagnostic criteria and evidence-based treatment of tension headaches.
6. Formulate an evaluation and treatment plan for patients with probable cluster headaches.

Day 3
Sensible Prescribing Practices for the Elderly. (Oberlender)
Upon completion of this session, the participant should be able to: EBM, COMP
1. Appraise and appreciate the limits of published trials studies on drug safety and efficacy for seniors.
2. Assess the subtle but significant drug side effects in the elderly.
3. Distinguish undesirable prescribing practices and employ better approaches based on current evidence-based referenced literature.

Osteoporosis: Treatment and Prevention. (Muncie)
Upon completion of this session, using the USPSTF, NOF Guidelines and the Cochrane Abstract Database, the participant should be able to: EBM, GL, COMP
1. Debate the importance of the risk factors for an osteoporosis fracture.
2. Employ the diagnostic testing for patients at risk with osteoporosis.
3. Identify the patients at highest risk for an osteoporosis related fracture.
4. Select the therapeutic medication to treat osteoporosis based upon the indications and side effects.
5. Assess the therapeutic role of repeating the DXA scan in patients treated for osteoporosis.

A Practical Approach to Falls and Urinary Incontinence in the Elderly. (Oberlender)
Upon completion of this session, the participant should be able to: EBM, GL, COMP
1. Integrate the concept of multifactorial causes underlying falls and employ the American Geriatrics Society Guidelines for the Prevention of Falls and urinary incontinence in seniors.
2. Develop a full assessment approach to evaluate different causes, using detailed physical examination and functional performance evaluations.
3. Differentiate the effects of normal aging from pathologic processes and adverse drug effects as risk factors for falls and incontinence.
4. Apply NIH guidelines to formulate a preventative and therapeutic plan of care.

Treatment of Sexually Transmitted Infections. (Muncie)
Upon completion of this session, using the CDC Guidelines, the USPSTF guidelines and the Cochrane Abstract Database, the participant should be able to: EBM,GL, COMP
1. Determine the appropriate diagnostic testing for complaints suggestive of a sexually transmitted infection.
2. Select the therapy for the commonly encountered sexually transmitted infections.
3. Formulate the follow-up testing and evaluations for sexually transmitted infections.
4. Recommend the therapeutic options for preventing herpes simplex infections.
5. Identify when and how to use expedited partner treatment for select sexually transmitted infections.
Improving Communication Skills in Medical Practice. (Oberlender)
Upon completion of this session, the participant should be able to: COMP
1. Appraise the variety of ways that people perceive communication and process information.
2. Assess your personal communication strengths and limitations.
3. Determine communication issues unique to older patients.
4. Develop practices and apply skills to maximize effective communication and listening based on current literature.
