NARRATIVE DESCRIPTION
Following this course, the participant should be able to appraise the etiologies and recognize the clinical presentations of the commonly seen disorders presented; indicate diagnostic tests available and the suggested use for making a diagnosis; implement available treatment options and appropriate long-term care when indicated. This activity is expected to result in improved competence in making an appropriate diagnosis and providing effective treatment and referral or follow-up care with the overall goal of improving patient outcomes.

When indicated in the specific objectives, emphasis will be on aligning physician behavior with current guidelines and evidence-based medicine with a focus on diagnosis, treatment and when to refer. This course was designed to inter-relate the disorders presented in an effort to help all practitioners at the level of a practicing physician or practitioner relate their practice strategies with other providers and specialists. By doing this, it will provide them with increased competence to appropriately diagnose and refer or effectively treat their patients’ conditions to optimize their care.

SPECIFIC OBJECTIVES
Day 1
Adrenal Insufficiency- Primary and Secondary. (Winters)
Upon completion of this session, using evidence-based medicine and clinical practice guidelines, to include Endocrine Society Guidelines, the participant should be able to: COMP, EBM, GL
1. Apply principles learned to evaluate the diagnostic tests used to confirm the diagnosis or primary or secondary adrenal insufficiency.
2. Learn the causes of primary and secondary adrenal insufficiency.
3. Employ therapeutic options for patients with primary or secondary adrenal insufficiency.

Practical Approach to the Patient with Headache. (Horowitz)
Upon completion of this session, while applying recommendations from the IHC, NHF, AAFP, AAN and ACP, the participant should be able to: GL, COMP
1. Review the differential diagnosis of a patient presenting with the complaint of headache.
2. More confidently evaluate the patient with headache and appreciate the indications for imaging.
3. Develop a treatment plan that considers lifestyle changes as well as medications.

Cushing Syndrome and other Functional Disorders of the Adrenal. (Winters)
Upon completion of this session, using evidence-based medicine and clinical practice guidelines, to include Endocrine Society Practice guidelines, the participant should be able to: COMP, EBM, GL
1. Outline the etiologies of Cushing syndrome.
2. Evaluate a patient with an adrenal tumor.
3. Evaluate a patient for suspected hyperaldosteronism.
4. Employ strategies to appropriately diagnosis and manage patients with pheochromocytoma.

Practical Approach to the Patient with Diabetes. (Horowitz)
Upon completion of this session, using multiple research trials as well as recommendations from the ADA, EASN, AGA, CDA, AACE, ACP, and the AAFP, the participant should be able to: GL, COMP
1. Consider this primer on management.
2. Distinguish the patient who is likely to go on to develop complications and appreciate the potential long-term course.
3. Apply the current guidelines for therapy and access the anticipated outcome goals.

Thyroid Case Studies. (Winters)
Upon completion of this session, using evidence-based medicine and clinical practice guidelines, to include the American Thyroid Association Guidelines, the participant should be able to: COMP, EBM, GL
1. Appreciate the role of a diagnostic thyroid scan or ultrasound.
2. Evaluate a patient for hyperthyroidism.
3. Prescribe therapeutic options for patients diagnosed with hypothyroidism.

Day 2
Polycystic Ovary Syndrome. (Winters)
Upon completion of this session, using evidence-based medicine and clinical practice guidelines, to include Rotterdam Guidelines, the participant should be able to: COMP, EBM GL
1. Recognize the distinguishing features of PCOS.
2. Prescribe appropriate laboratory tests to evaluate women with hirsutism.
3. Describe the non-reproductive aspects of PCOS.

Approach to the Management of Chest Pain. (Horowitz)
Upon completion of this session, the participant should be able to: GL, COMP
1. Evaluate the complaint in the context of history, physical exam, and EKG findings.
2. Develop a strategy to use cardiac biomarkers in the evaluation of ACS.
3. Use recent research studies and guidelines to develop an approach to the patient with chest pain.

Male Hypogonadism. (Winters)
Upon completion of this session, using evidence-based medicine from the JCEM, NEJM and others, the participant should be able to: COMP, EBM
1. List the classic causes of male hypogonadism.
2. Appreciate the entity of adult onset hypogonadism.
3. Describe the risks and benefits of testosterone use and apply toward decision making when reviewing treatment options.

Practical Approach to the Patient with Dementia. (Horowitz)
Upon completion of this session, while applying recommendations from the AAN and the USPSTF, the participant should be able to: GL, COMP
1. Formulate a practical approach to screening for dementia.
2. Discuss medication management to include polypharmacy, drug-drug interactions, as well as new drugs and their potential efficacy and side effects.
3. Develop a long-term follow-up and referral process that considers cost-effective and resource management.

Disorders of Calcium Metabolism. (Winters)
Upon completion of this session, using evidence-based medicine from the JCEM, Annual of IM and others, the participant should be able to: COMP, EBM, GL
1. Distinguish the causes of hypercalcemia.
2. Recognize secondary hyperparathyroidism.
3. Be aware of the indications for surgical intervention using clinical practice guidelines for asymptomatic hyperparathyroidism.
4. List the causes of hypocalcemia.

Day 3
Orthopedic Injuries and Problems. (Zun)
Upon completion of this session, the participant should be able to: COMP
1. Detect various orthopedic injuries and ailments.
2. Formulate a treatment plan for orthopedic injuries.
3. Demonstrate the application and use of splints for orthopedic injuries.

ENT Disorders. (Zun)
Upon completion of this session, the participant should be able to: COMP
1. Distinguish various eye, face, ear, nose, neck and upper airway disease processes.
2. Determine the proper treatment for ENT emergencies.
3. Describe the means to remove foreign bodies from ear, eye and nose.

Anxiety Disorders. (Zun)
Upon completion of this session, the participant should be able to: COMP
1. Understand the burden and incidence of anxiety disorders.
2. List the diagnostic criteria for various anxiety disorders.
3. Review treatment options for common anxiety disorders.

Practical Approach to the Patient with Potential DVT. (Horowitz)
Upon completion of this session, using multiple research trials as well as applying recommendations from the ACCP, AAFP/ACP, and the AHA/ACC, the participant should be able to: GL, COMP
1. Obtain an appropriate medical history to better identify those patients at risk.
2. More confidently evaluate and assess the patient suspected of having DVT.
3. Better manage patients with the confirmed diagnosis of DVT.

Strategies to Address the Issue of Preventable Hospital Readmissions. (Horowitz)
Upon completion of this session, the participant should be able to: EBM, COMP
4. Identify the factors which contribute to increase risk of readmission.
5. Stratify the risk for readmission utilizing evidence-based tools.
6. Develop a transition of care strategy to reduce the likelihood of a preventable readmission.

Day 4
Adult ADHD. (Zun)
Upon completion of this session, the participant should be able to: COMP
1. Describe the diagnostic criteria for ADHD.
2. List pharmacologic and non-pharmacologic treatment options.
3. Formulate the risks and benefits of ADHD treatment (and lack of treatment).
4. Employ several psychosocial interventions for adults with ADHD.

Depressive Disorders. (Zun)
Upon completion of this session, the participant should be able to: COMP
1. Detect the signs and symptoms of depression.
2. Assess presentations of depression across the lifespan.
3. Employ pharmacological and non-pharmacological treatment options for depression.

Practical Approach to the Patient with Hyperlipidemia. (Horowitz)
Upon completion of this session, using multiple research trials as well as the recommendations of the NCEP, the participant should be able to: GL, COMP
1. Apply the current NCEP guidelines.
2. Develop a practical approach for considering the new medications available.
3. Consider the new targets developed for cholesterol end point.

Non-Surgical Approaches to the Patient with Obesity. (Horowitz)
Upon completion of these sessions, using multiple research trials as well as applying recommendations from the AGA, NIH – NHLBI, ACP, AHA, ASBS, and ACS, the participant should be able to: GL, COMP
1. Review the current popular diets available and marketed to your patients.
2. Take into consideration unique needs of the obese patient in order to create a comprehensive approach to the treatment of obesity.
3. Explain the basic physiology of obesity.
4. Manage the medications available for the treatment of obesity.

Approach to the Management of Stroke. (Horowitz)
Upon completion of this session, the participant should be able utilize the latest evidence based medicine and guidelines to: EBM, GL, COMP
1. Develop a strategy to assess the common presentations of stroke/TIA.
2. Utilize current initial treatment strategies appropriately.
3. Develop a strategy to stratify the risk of stroke.
4. Develop a secondary prevention plan.
