Target Audience

This activity is designed to meet the educational needs of physicians, neurologists, geriatricians, psychiatrists, nurse practitioners, physician assistants and nurses.

Program Overview

Type 2 Diabetes Mellitus (T2DM) is a chronic medical condition which poses serious health risks to older patients in long-term care facilities. Through this gap-based activity, learners will be able to skip content they know well and view content that is consider essential by the faculty or they have a demonstrated gap in knowledge.

Learning Objectives

Upon completion of this activity, participants should be better able to:

Individualize A1C goals based on patient- and disease-specific factors
Compare and contrast fixed-ratio combinations of GLP-1 receptor agonists (RA) plus basal insulin with other insulin regimens
Devise glucose-lowering regimens that optimize quality of life and glucose control and minimize hypoglycemia
