Target Audience

This activity is designed to meet the educational needs of primary care clinicians.

Program Overview

This activity will cover the importance of continuous glucose monitoring (CGM), the selection of appropriate patients for CGM, the types of devices currently available or under FDA review, and how to determine which options are best for the individual patient (including a discussion of Medicare, Medicaid, private insurance coverage and options for the patient with no insurance). Additionally, the discussion examines how to collect the resulting data from CGM, a case study illustrating the use of that data to make informed treatment decisions with the patient, and the role of CGM in empowering the patient. Billing and coding for CGM are also covered. The final segment features a patient detailing the impact of CGM on her diabetes management, and a faculty conversation about key points in the presentation.

Learning Objectives

Upon completion of this activity, participants should be better able to:

Identify patients who are good candidates for CGM versus fingerstick blood glucose monitoring.
List the types of information provided by CGM systems.
Identify and interpret CGM data using the AGP to assess if the patient is achieving targets established by the International Consensus on Time in Range.
Modify the treatment plan based on CGM data to improve patient outcomes.
