Clinical Consultations: Improving Outcomes By Addressing Health Care Disparities in Heart Failure Care

Overview / Abstract:

Target Audience
This educational activity is directed toward cardiologists. Primary care providers, nurses, nurse practitioners, physician assistants, and other clinicians involved in the management of patients with heart failure are also invited to participate.

Program Overview
This Clinical Consultations™ is a video-based program in which 2 experts discuss health disparities that negatively impact patients with heart failure. Access to care, insurance costs, and unintended bias are just some of the factors that contribute to the existence and persistence of these disparities. This engaging format is designed to help clinicians better recognize and take steps to address ethnic and racial disparities in heart failure, thereby forwarding progress to improve patient outcomes in heart failure.

Learning Objectives
Upon completion of this activity, participants should be better able to:
Describe the burden of existing healthcare disparities in heart failure care
Apply the new universal definition and classification to the management of heart failure to at- risk populations
Identify the determinants of health care disparities resulting in suboptimal patients care and poor outcomes
