Streamlining Preventive Health for Patients with IBD Through Effective Care Models, Evidence-based Guidelines, and Patient Engagement

Overview / Abstract:

The Cleveland Clinic estimates that less than 20% of patients with IBD seen in their Digestive Disease & Surgery Institute have received appropriate immunization and preventative services that align with national guidelines and recommendations. Education is an important component for creating a roadmap and gaining buy-in among clinicians to implement change in practice. Educational interventions to both healthcare professionals (HCPs) and patients as part of the project to improve patient health, vaccination rates, and preventive services are essential for these changes.

Outside of the clinic, digital health coaching presents additional opportunities to supplement and support patient education and reinforce the need to complete all necessary vaccinations among a group of 100 patients. Patient-reported outcomes and feedback will be collected to assess improvements in vaccination and preventive health literacy, communication with providers, and shared decision-making.

In this CME Outfitters Live and On Demand, expert faculty will discuss best practices for vaccinations and preventive services among individuals with IBD along with resources that will help clinicians to truly engage patients.
